The respective rates are calculated as shown below, where I is a completed interview, P is a partial interview, R is a known refusal, NC is a noncontact (someone referred by a parent respondent and selected by the NSM algorithm who failed to respond to the research team's invitation after several contact attempts), and O is other (the parent respondent provided invalid referral information on the alter).
